20 HOURS IN-CLASS AND 10 HOURS IN-VEHICLE INSTRUCTIONS MUST BE COMPLETED WITHIN ONE YEAR OF COURSE START DATE, WITH A PASSING MARK OF 70%. IN-CLASS INSTRUCTIONS MUST COMMENCE BEFORE IN-VEHICLE INSTRUCTIONS. DATES AND TIMES OF IN-VEHICLE INSTRUCTIONS ARE TO BE ARRANGED WITH THE ASSIGNED INSTRUCTOR. IN-VEHICLE INSTRUCTIONS MUST BEGIN AND END AT HOME, LESSONS STARTING AND ENDING AWAY FROM HOME MUST BE AUTHORIZED. I CERTIFY THAT THE STATEMENTS IN THIS DOCUMENT ARE ACCURATE AND GIVE CONSENT TO BELL DRIVER EDUCATION INC TO A PHOTO COPY OF THE ONTARIO DRIVERS LICENCE AND THE RELEASE OF ANY INFORMATION CONTAINED HEREIN TO THE MINISTRY OF TRANSPORTATION OF ONTARIO, THE INSURANCE BUREAU OF CANADA AND THE MTO COURSE INSPECTOR.

Step 4 — Payment Options

Review your payment options below. A $250.00 deposit is due upon registration. You'll be able to complete payment after submitting this form.

Option 1: E-Transfer

Send e-transfer deposit to info@belldriver.ca. Please use the student's full name in the message box.

Option 2: In Person Payment

Cash, check, or other methods accepted by appointment at our office: 686 Wilson Ave, Toronto.
